Reviewing and Updating Care Notes

Learn how office staff can review task responses, care summaries, and signatures on a shift's Care tab, correct care documentation, and download care notes for one shift or a date range.

Everything a caregiver records during a visit is saved on the shift. This includes their responses to care plan tasks, the care summary, and signatures.

Office staff can review this information on the shift's Care tab in the web app.

Open the shift's Care tab

  1. In the left sidebar, under Operations, click Shifts.

  2. Find the shift on Table, Calendar, or Timeline, then click it.

  3. Near the bottom of the shift page, click Care. This tab is between Activity and History.

The Care tab becomes available when the caregiver clocks in and the shift is In Progress. It stays available after the shift is completed.

If no one clocked in, the tab stays grayed out. This includes Incomplete or Canceled shifts that were never clocked in.

What the Care tab shows

Section

What you will see

Verification

The client's and caregiver's signatures. If the client could not sign, it shows Unable to sign. This section appears only when at least one signature was captured.

Summary

The caregiver's care summary for the visit. If it is blank, you will see "An empty care summary has been provided for this shift."

Tasks

Each recorded task, its status (Completed, Declined, or Not Needed), the time the response was recorded, and any note.

Keep these details in mind:

  • Only tasks with a recorded response appear. Tasks the caregiver did not respond to are not listed. To see every care plan task, use the Care Note PDF. Tasks without a response appear there as Not Completed.

  • The time beside a task shows when the response was recorded. It may not be the time the care happened. For example, a task added by office staff after a visit shows the time it was added.

  • Tasks come from the client's signed care plan for that service. See Intake, Assessments, and Care Plans.

  • Clock-in and clock-out times and locations, mileage, and expenses appear elsewhere on the shift page and in the Care Note PDF. They do not appear on Care.

Correct care documentation

Once a shift is Completed, staff with permission to manage shifts can update its documentation on Care.

Update the summary

Click Edit next to Summary to correct the care summary.

Add, edit, or delete a task response

  • To record a missed task, click Add. Choose Complete, Declined, or Not Needed, and add any Notes.

  • To change an existing response, open the task's ⋮ menu and choose Edit or Delete.

Office staff must wait until the shift is completed before making these changes. While it is In Progress, ask the caregiver to update their documentation.

Changes made by office staff are recorded on the shift's Activity tab. For example, it may show "…edited a care task result for Bathing."

Caregivers can also update their own documentation after a shift is completed, using the mobile app or caregiver web portal. They can add, edit, or delete task responses and edit the summary. In the mobile app, they can also add missing required signatures. See Using the Caregiver App.

Export care notes

One shift: Care Note (PDF)

  1. Open a completed shift.

  2. Click Export in the top action bar, then choose Care Note.

  3. In the preview, click Download to save the PDF. Or click Add to Client Documents to save it on the client's Documents tab.

Care Note is available only after the shift has been added to an invoice or claim. Until then, it is grayed out. The tooltip mentions only invoices, but claims also count.

The Care Note PDF includes:

  • Your agency's details.

  • The client, caregiver, and date of service.

  • Clock-in and clock-out times, locations, and EVV status.

  • Every care plan task, with its status and notes.

  • The care summary.

  • Signatures, based on the payer's requirements.

Before adding a PDF to Client Documents: The client's guests, such as family members, can see documents saved there. You need permission to manage client documents to use Add to Client Documents.

A date range: Care Notes (PDF) or Spreadsheet (CSV)

  1. On Shifts, open Table and set the date range.

  2. Click Export.

  3. Choose Care Notes (PDF) or Spreadsheet (CSV).

Export option

What it includes

Care Notes (PDF)

Every completed shift in the date range, grouped by client. For each client and service, the report includes a weekly shift table with times, caregiver, hours, miles, and signatures; a day-by-day task grid; care summaries and task notes; and totals. You can download each client's report or add it to their documents.

Spreadsheet (CSV)

The list of shifts shown in the table, using your table filters.

Keep these export rules in mind:

  • Care Notes uses only the date range and supports up to 90 days. It ignores other table filters, including caregiver and status.

  • Spreadsheet follows your table filters.

  • For date-range exports, Export is available on Table, not Calendar or Timeline.

  • To export care notes for a specific invoice, use Care Notes on that invoice. See Creating, reviewing, and sending invoices.

Frequently asked questions

Why is the Care tab grayed out?

The shift has not been clocked in to. The Care tab becomes available when the shift is In Progress. It stays grayed out for shifts that were never clocked in, including Incomplete or Canceled shifts.

If the caregiver worked but could not clock in, an office user can complete the shift manually. See Clock-in and clock-out troubleshooting.

Why is a care plan task missing from the Care tab?

Care lists only tasks with a recorded response. If the caregiver did not respond to a task, it will not appear.

Once the shift is completed, use Add to record the missing response. You can also check the Care Note PDF, where tasks without a response appear as Not Completed.

Can a missed task or incorrect note be fixed?

Yes. Once the shift is completed, staff with permission to manage shifts can edit the summary and add, edit, or delete task responses on Care.

The caregiver can also update their own documentation in the mobile app or caregiver web portal.

Why is Care Note grayed out in Export?

The single-shift Care Note becomes available after the shift is added to an invoice or claim. Add it to an invoice or claim first. See Add to Invoice vs. Add to Claim.

To review documentation before then, use Care or the date-range Care Notes export.

Can caregivers be required to complete tasks or a summary before clocking out?

Yes. On the client's care plan, turn on:

  • Require Responses To All Care Tasks.

  • Require Care Summary.

  • Require Caregiver Notes on individual tasks.

Caregivers cannot clock out in the app until they complete the required documentation.

Set signature requirements under Settings → Policies → Shift Signatures. See Setting up client and employee signatures at clock-out.

Can family members see task responses?

Family members and other guests can see tasks and their status on completed shifts. They cannot see task notes or the care summary in that view.

They can also see care note PDFs you add to the client's Documents tab. See Managing Client Connections & Guest Access.

Can Looper review care documentation?

Yes. Open the shift and click More → Ask Assistant to ask for a visit summary. You can also ask the Assistant to look for missing documentation across shifts.

AI Shift Review rules can flag missing tasks, notes, or summaries. See How AI Shift Review Works.

Where do caregivers enter care documentation?

Caregivers enter it during the shift in the Careswitch mobile app or caregiver web portal. See Using the Caregiver App.
